Mandatory Program Forms
For your child to attend programming at the Nature Center, you must complete and submit the forms required for that program.
Please log in to your Playground account and navigate to the Paperwork section to find the required medical forms for your child's program.
Ways to Submit Completed Forms:
- Online via your Playground account
- By Email to registration@dariennaturecenter.org
- By Mail to the Darien Nature Center at 120 Brookside Road, Darien, CT 06820
- By Hand to the front office at the Nature Center!
For All Caregiver & Child Preschool Programs
Includes Babes in the Woods
If your child has allergies, asthma or other conditions that require medications to be on the premises, we ask that you provide an Emergency Action Plan for each medication. Caregivers are responsible for the administration of any medication, but staff need to be aware of any action plan.
