Mandatory Program Forms
For your child to attend programming at the Nature Center, you must complete and submit the forms required for that program.
Ways to Submit Completed Forms:

For All Caregiver & Child Preschool Programs
Includes Babes in the Woods and Knee High to Nature
If your child has allergies, asthma or other conditions that require medications to be on the premises, we ask that you provide an Emergency Action Plan each medication. Caregivers are responsible for administration of any medication but staff needs to be aware of any action plan. Below are samples, submitted forms must be completed by your child's doctor.
For All Drop-Off Preschool Programs
Includes Nature Time for Twos, Nature's Playroom, Critters & Kids, and Our Living Planet
If your child has allergies, asthma or other conditions that require medications to be on the premises, you must also provide an Emergency Action Plan and Authorization for the Administration of each medication. Below are samples, submitted forms must be completed by your child's doctor.
For All After School Programs
Includes Animal Encounters, Animal Caretakers, Outdoor Explorers, Out in the Wild, Water Ecology, and Geology Rocks

For All Vacation Camps and Special Programs
Incudes Groundhogs, Crickets, Chipmunks, and Snowbirds

- Childhood Assessment Form (choose one)
- Early Childhood Health Assessment Record (birth to 5 years)
- Childhood Health Assessment Record ( ages 6+)
